Os Trigonum Syndrome
Os Trigonum Syndrome, also known as Dancerís Heal or Posterior Ankle Impingement Syndrome (PAI Syndrome), is common to ballet dancers, gymnasts, and football kickers. Os Trigonum Syndrome occurs when the tissue at the back of the ankle between the talus and tibia is pinched. This pinching or compression is caused by a bony formation, called an os trigonum, at the back of the ankle. An os trigonum is congenital (present at birth) but only exists in 5-15% of the population. Often, people will not know they have an os trigonum unless it begins to cause pain. The pain may be triggered by an ankle sprain, tendonitis or by hyper plantar flexion (pushing the ankle forward). At times, the os trigonum may become trapped between the ankle and heal bone. When the soft tissue at the back of the ankle is pinched it becomes inflamed and painful. At times, the os trigonum will pull away from the talus bone and the tissue that connects it to the talus becomes stretched or torn. This also causes the area to become inflamed and painful.
Suffers of Os Trigonum Syndrome will experience pain, tenderness and swelling at the back of the ankle. The pain becomes worse when the toes are pointed downward (plantar flexion).
The RICE (Rest Ice Compression Elevation) method has long been a tried and true treatment for painful inflammation. The best way to heal is to reduce swelling and rest the ankle, the RICE method facilitates this as well as a quicker healing time.

When it comes time for more aggressive rehabilitation, stretching and activity, heat therapy warms the muscles in the injured area which increases range of motion and allows for a safer more thorough stretch.
It should be stressed here that in the initial stages of tissue damage you should avoid treatments that will increase swelling such as hot showers, heat rubs, hot packs and consuming alcohol. During the initial phases of rehab the ankle should not be moved in any direction that mimics the direction in which the injury occurred.
