Calcaneal Apophysitis, also called Sever's Disease or Achilles apophysitis, is a children’s disease that causes pain in the back of the ankle where the Achilles tendon attaches to the heel. When children undergo growth spurts, between the ages of 7 and 15, the bones often grow faster than muscles and tendons. As a result, the bones get longer and the muscles and tendons become tighter. During activities that put weight on the heel, the tight heel tendons put too much pressure on the back of the heel, injuring the growth plate and causing Calcaneal Apophysitis.
Children most at risk for Calcaneal Apophysitis include active girls, aged 8 to 10, and active boys, aged 10-12. Any children who overuse the heel during running or jumping activities can be susceptible to Calcaneal Apophysitis. It occurs frequently in gymnasts as well as basketball, soccer, tennis and baseball players.
Calcaneal Apophysitis may be prevented if a child maintains good flexibility by stretching the hamstring, calf muscles and leg tendons during growth. Not going barefoot and wearing shoes with firm support and shock-absorbent soles will also help.
Children with Calcaneal Apophysitis will experience pain in one or both heels. Usually the pain increases when the child puts pressure on the heel (running or jumping) or if you squeeze the heel from the sides. Often the pain is noticed when activity increases at the beginning of a new sports season or if the child takes up a new sport. The child may limp due to the pain. Flexibility of the ankle joint may be limited and the child could have a tendency to tiptoe due to the tightened tendon.
A child suffering from Calcaneal Apophysitis should stop or minimize any activity that causes the pain. Calcaneal Apophysitis will go away on its own when the bone is finished growing but cold compression and heat therapy will help to sooth the pain in the duration. Symptoms usually subside within 2-8 weeks but proper care is important to prevent injury when the tendon is tight.
The RICE (Rest Ice Compression Elevation) method has long been a tried and true treatment for inflammation. The best way to heal is to reduce swelling and rest the ankle, the RICE method facilitates this as well as a quicker healing time.

When it comes time for more aggressive rehabilitation, stretching and activity, heat therapy warms the muscles in the injured area which increases range of motion and allows for a safer more thorough stretch.
It should be stressed here that in the initial stages of tissue damage you should avoid treatments that will increase swelling such as hot showers, heat rubs, hot packs and consuming alcohol. During the initial phases of rehab the ankle should not be moved in any direction that mimics the direction in which the injury occured.
